Due to the number of different services we have available, it is best to call our office at (513) 563-0488 and press extension 1 to speak with the Intake Department. However, you can also complete the form below. Once received, our intake coordinator will reach out to you.
Our main office in Sharonville is open from 8:00 AM to 5:00 PM Monday to Thursday, and 8:00 AM to 4:00 PM on Friday. We are available by phone during those hours as well. We are closed on the weekend. Appointment schedule hours vary at each office location. Please call to get more details.
